Diabetes Management
Middleton Family Medicine is an American Diabetes Association Certified Program.
The Diabetes team includes Erica Ouellette, M.Ed., RD, CDCES, Angela Fortenbacher, FNP, CDCES and Rachel Riley, NP, CDCES & Jennifer Braimon, MD.
Individual and group counseling appointments are offered via telehealth and in the office.
Our team is up to date on the latest technology in terms of Continuous Glucose Monitors and Insulin Pumps as well as download options and ongoing support.
Our 8 week Diabetes Group Education Series is offered via Zoom and is a rolling admission.
Nutrition Counseling
Our Registered Dietitian, Erica Ouellette, M.Ed., RD, CDCES, is available to consult with you on your personalized nutrition needs including:
