Welcome to our practice!

If you’re transferring from another provider, please request that your records be sent to us to ensure the most efficient and helpful visit. Download the request form to provide to your previous office.

For our Existing Patients

Middleton Family Medicine has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.

How to Submit a Request

  1. Visit requestmanager.healthmark-group.com.

  2. Once logged in, select “Submit Request” from the menu options

  3. Enter all required fields to provide an authorization directly to HealthMark.

 

Your medical record request will be processed, and a notification will be sent via mail or email once complete and available for download.
